Clinical Pharmacist Vacancy

Cornerstone Practice, Blackburn is looking to recruit a new Clinical Pharmacist post to serve over 22,000 patients across Blackburn. The post holder will be based at our Shadsworth Surgery, just 5 minutes from Junction 5 of the M65, but will be required to travel to other sites in Blackburn and Oswaldtwistle on occasion.

This brand new role is an exciting opportunity to enhance pharmacy care within our forward thinking and friendly general practice as part of an innovative NHS England Development Programme.

It is ideally a full time (37.5 hours per week) post, but minimum of 30 hours per week could be considered.

Agenda for Change Band 7, (point depending on experience/qualifications).

The successful candidate will be an experienced pharmacist, who acts within their professional boundaries, to work in a general practice environment to support the care of patients, interact with other local pharmacists and a pharmacist mentor to ensure continuous educational development. Ideally the candidate would also have experience of working within a Primary Care environment.

Other aspects of the role will include:

Working in a multi-disciplinary team in a patient facing role Integrate with community and hospital pharmacists

Being a competent independent prescriber or be willing to develop this under a structured education plan

Primary support to practice staff regarding prescription and medication queries Providing expertise in clinical medicines

Responsibility within complex polypharmacy and chronic disease patients

Knowledge of Emis Web and General Practice links and pathways would also be advantageous.
